I hear by give consent for my child, named above, to receive the piercing stated by Pretty Pierced in my absence.

I understand that Pretty Pierced are professional, licensed and insured and will only perform the service with my consent, with my ID and with my child's ID and, if is safe to do so.

I understand that Pretty Pierced will provide all the relevant aftercare guidelines and support to my child, for them to carry out under my care. They are to return for their free health check in 8 weeks and contact Pretty Pierced with any concerns or questions

If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.

Please take photo(s) of your government issued photo IDs and related paperwork
To provide that you are the legal consenting adult, we require 3 things!!

1. A document to prove guardianship eg:

- Birth certificate with both child and your name on it
- Adoption certificate
- Child maintenance with both names
- Foster documents with foster and parental consent

If your names differ from child or original document, please provide a document showing name change. EG
- Marriage certificate
- Divorce certificate
- Deed-poll certificate


2. We also need a photo of your photo ID EG
- Driving licence
- Passport

3 - A selfie
The idea is we need to match child to you legally. No cool big sisters or funky Aunties or naughty Nans. This is to safely prove that legally you are responsible for the child to have s skin piercing procedure. I do not want angry parents calling me up thanks!
