1. I am at least 18 years old and have proper legal identification to prove it. ID will be copied on the back of this form.
2. If I have a heart condition, diabetes, epilepsy, hemophilia, HIV, AIDS, Hepatitis, or any other condition, OR if I am taking ANY medication, my artist has been informed and a note concerning the details of the condition and/or medication has been made on the back of this form.
3. I am not currently under the influence of any drugs, alcohol, or substances that affect judgment.
4. I do not have any physical or mental disabilities that may affect my well being as a result of my decision to have a tattoo/piercing.
5. I agree to follow the care instructions given to me. I am aware that I am responsible for the healing process and overall outcome of the healing of the tattoo/piercing.
6. I agree that the outcome of the tattoo/piercing is dependant on how it is cared for. I also understand that various properties of skin types can affect the healing process and know that there is no guarantee that any particular tattoo/piercing colors will hold and that this is not due to any fault of the artist.
7. Being of sound mind and body, I hereby release any and all persons representing the Neon Dragon Tattoo Parlor from all responsibilities. I accept any and all responsibility for ANY consequences which may stem from my decision to get a tattoo/piercing.
8. I hereby release Jason Evans and the Neon Dragon Tattoo Parlor from ANY and all problems that could occur from having a tattoo/piercing done.
9. I agree for myself, heirs, assigns, and legal representatives to hold the Neon Dragon Tattoo Parlor, Jason Evans, or anyone else associated with them harmless from all damages, actions, causes of action, claim judgments, cost of any litigations, attorney fees, along with any and all costs or expenses which may arise from my decision to have a tattoo/piercing done, and that I any litigation that is brought will be litigated in Neon Dragon's home jurisdiction regardless of what rights I may have to litigation in my own home jurisdiction.
10. By releasing responsibility I realize that I am agreeing not to sue or harass anyone in connection to any damages, claims, demands, rights, and causes of action whatever kind in nature based upon injuries or property damage to, or death of myself or any other persons arising from my decision to have a tattoo/piercing whether or not caused by any negligence of the artist.
11. I understand that by getting a tattoo/piercing, I am purposefully creating an open wound on my body. I understand that any open wound can become infected or scarred and may require antibiotics or medical treatment if not properly taken care of. I understand that no expenses will be paid by Jason Evans, Neon Dragon Tattoo/piercing Parlor; its owners, operators, employees, or agents. Any expenses incurred are completely my responsibility as part of my decision to get a tattoo/piercing done. I understand that I may faint or lose consciousness during or after getting a tattoo/piercing and accept this risk as well as any medical expenses that could occur from that. I understand that there is a possibility of allergic reactions to the ink, alcohol, soaps, latex gloves, and other materials used while doing a tattoo/piercing. I agree that this is not the fault of the establishment if I have an allergic reaction.
12. I agree and understand that any expenses that may incur from getting a tattoo/piercing will be my sole responsibility and not that of the persons or establishment where the tattoo/piercing was performed. I therefore release Neon Dragon Tattoo Parlor; its owners, operators, employees, contractors, and agents from any and all liability regarding my getting a tattoo/piercing or tattoo/piercing related work at the Neon Dragon.
13. I agree to follow the care instructions and not to hold Neon Dragon Tattoo Parlor; its owners, operators, employees, contractors, and agents responsible for any medical bills associated with this. All bills will be at my own expense.
14. I understand that Neon Dragon Tattoo Parlor; its owners, operators, employees, contractors, and agents do not represent themselves to be trained in any medical field. If I have a medical concern regarding my tattoo/piercing such as redness or inflamed tissue, I will seek qualified medical attention and I will do so at my own costs.
15. I understand that there are risks associated with getting a tattoo/piercing. I agree to be solely responsible for any financial loss incurred, including but not limited to medical bills, loss of work, permanent disability, and pain and suffering.
16. I know that all sales are final.
17. We do reserve the right to perform our own artistic interpretations of any or all tattoo/piercing artwork.
18. I understand that although tattoos are permanent, they can fade. Many variables can contribute to the fading of a tattoo such as the sun, how it is cared for during healing, or even just the skin type. Some of these things are beyond the control of the artist. We will, at not cost, touch up the tattoo if it needs it within 3 months.
