That I have been fully informed of the inherent risks, associated with getting a tattoo. I fully understand that these risks, known and unknown, can lead to injury, including but not limited to infection, scarring, difficulties in detecting melanoma and allergic reactions to tattoo pigment, latex gloves, and/or soap or aftercare balms. Having been informed of the potential risks, I understand that any lifestyle choices or job types may impact on the potential risks such as working in a dirty or dusty environment. I still wish to proceed with the tattoo application and I freely accept and expressly assume any and all risks.
WAIVER

TO WAIVE AND RELEASE to the fullest extent permitted by law each of the Artist and the Studio from all liability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors or assigns may have for personal injury or otherwise, including any direct and/or consequential damages, which result or arise from my tattoo, whether caused by the negligence or fault of either the Artist or the Tattoo Studio, or otherwise.
HEALING
*
The Artist and the Tattoo Studio have given me instructions on the care of my tattoo while it's healing, and I understand them and will follow them. I have also been offered to purchase recommended aftercare products. I acknowledge that it is possible that the tattoo can become infected, particularly if I do not follow the instructions given to me.
DURESS
*
I am not under the influence of alcohol or drugs, nor am I being coerced into getting tattooed by another person. I am voluntarily submitting to be tattooed by the Artist without duress or coercion. If you are not safe with the person that is accompanying you and you need help then please ask your artist if you they know an Angela Jones.
SPELLING & FONT STYLE

(PLEASE PLEASE TRIPLE CHECK ANY SPELLING AND NUMBERS/DATES!)Neither the Artist nor the Tattoo Studio is responsible for the meaning or spelling of the symbol or text that I have provided to them or chosen from the flash (design) sheets nor are they responsible for any online searches they have made on my behalf to find a specific number, spelling or design. I understand that some font styles can cause certain letters to appear differently, making the chosen word appear to be spelt differently or incorrectly.

HEALTH & ILLNESSES

Do you have any health issues or illnesses such as but not limited to, diabetes, epilepsy, hemophilia, a heart condition, autoimmune disease. Do you take any blood thinning medication or roaccutane. I do not have any other condition or take any medications that may interfere with the application or healing of the tattoo. I am not the recipient of an organ or bone marrow transplant or, if I am, I have taken the preventive anti-biotics. I am not pregnant or nursing. I do not have a mental impairment that may affect my judgment in getting the tattoo. I have received medical clearance from my GP to get Tattooed if any of the above apply to me.

FADING/FALLOUT
*
Variations in colour/design may exist between the art I have selected and the actual tattoo. I also understand that over time, the colors and the clarity of my tattoo will fade due to natural dispersion of pigment under the skin. I understand that certain areas of a the body are at higher risk of fading or complete fallout such as but not limited to, any part of the fingers/toes, hands/feet, knees/elbows. I understand that my lifestyle and job may impact on the healing and/or fading of my Tattoo such as excessive hand washing etc.
PERMENANT
*
A tattoo is a permanent change to my appearance and can only be removed by laser or surgical means, which can be disfiguring and/or costly and which in all likelihood will not result in the restoration of my skin.
LEGAL ACTION
*
I agree to reimburse each of the Artist and the Tattoo Studio for any attorneys' fees and costs incurred in any legal action I bring against either the Artist or the Tattoo Studio and in which either the Artist or the Tattoo Studio is the prevailing party. I agree that the that the courts of [Swansea] in [Wales] shall have personal jurisdiction and venue over me and shall have exclusive jurisdiction for the purpose of litigating any dispute arising out of or related to this agreement.
QUESTIONS

I acknowledge that I have been given adequate opportunity to read and understand this document, that any and all of my questions have been answered, that it was not presented to me at the last minute, and I understand that I am signing a legal contract waiving certain rights to recover against the Artist and the Tattoo Studio.
PHOTOGRAPHY
I release all rights to any photographs taken of me and the tattoo and give consent in advance to their reproduction in print or electronic form. (If you do not tick this provision, please advise your Artist).
BLOW OUTS
*
I understand that with any Tattoo I have on any part of my body, I am at risk of a blow out and I fully understand what a blow out is. This is when the pigment breaks down under the skin surface and potentially looks like bruising or ink blobs out of the lines and are permanent. Blow outs are more likely to occur in but not limited to thin skin areas such as fingers, hands, toes and feet or where a person has delicate, damaged or aged skin.
AFTERCARE
*
I have been given verbal and printed aftercare, I have also been offered to purchase the tattoo artists advised aftercare balm, soap and/or second skin, should I choose not to purchase any aftercare then I understand that my tattoo may not heal its best. I agree that I will follow the artists guidance for looking after my tattoo, should I not follow this guidance, I do so at my own risk.
BOTOX, FILLERS AND OTHER ENHANCEMENTS
I have not had you any fillers, Botox or other enhancements in the area I am being tattooed on within the last 3 months. I understand that there may be complications resulting from being pierced where the area does have un-dissolved fillers, Botox or other such enhancements. These complications include but arent limited to disfigurement, leakage and wide spread or internal infection.

NUMBING AGENTS
*
Have you used any numbing cream on the area you are being tattooed, have you discussed this with your artist. Numbing agents can cause the skin to become tough and rubbery, making it more difficult for your artist to tattoo. This can also cause issues with the healing such as your tattoo becoming patchy and faded in places. Numbing agents can also cause reactions in the skin for some people and this can be damaging to your tattoo as well as increasing the risk of infection. We do not sell or apply any numbing agents you may wish to use. Please read the instructions on application and risks carefully. Numbing cream can be over used and can lead to overdose or allergic reactions. Overdose Symtoms include, irregular heartbeat, seizures, difficulty breathing and could lead to possible death. If you choose to use numbing creams, you do so at your own risk and understand that your artist is not liable for any reactions or damage that may occur to your tattoo and as such there will be no free touch ups.
TOUCH UPS
*
I understand that should I need a touch up to my Tattoo, there will be set up fee of £30 for 30 minutes and then an additional fee of £30 for every 30 minutes thereafter. My touch up will not be free of charge unless my Artist has assessed the Tattoo and feels that it's thier responsibly that the tattoo has faded and this will be at their discretion.
PLACEMENT

I have thoroughly discussed the placement of my Tattoo with my artist and have listened to their advice on the best placement for my design and anatomy. Should I choose to go against my artists advice then I do so at my own risk and my artist is not liable to fix, cover, remove, refund any costs for correcting the Tattoo. I understand that ultimately the decision of placement is solely mine.
VISABLE TATTOOS

I understand the societal impacts of getting tattooed in visable areas such as, but not limited to face, head, neck and hands. I understand that I may be discriminated against, I may be unable to gain employment or it may impact future career or employment due to this.

TERMS & CONDITIONS

Do you agree to our T&Cs as outlined below.
Please make sure to eat a substantial meal before your appointment, you should eat at least 30 minutes before.We cannot stress enough how important it is to eat before getting your Tattoo, your body will be put under stress while being Tattooed so you'll need food for energy.
Make sure to stay hydrated, drink plenty of water the day before and the day of your Tattoo, this will not only help to keep you going but will also make a huge difference to your skin and how it reacts to the Tattoo, it will also help will the healing process.
Try to avoid alcohol and recreational drugs for at least 24 hours prior to your appointment as this can cause excessive bleeding which can make it harder for your skin to absorb the ink. If you are under the influence of alcohol or drugs at the time of your appointment, we will not Tattoo you and you will lose your booking fee.
You are more than welcome to bring a drink and snacks to get you through your appointment, there are shops and take always in the area.
We recommend exfoliating the area a day or 2 before, this helps remove dead skin cell build up, it can make the Tattoo procedure more comfortable and can give a better healing result.
Do not shave or moisturise the area of the Tattoo within 24 of your appointment, as this can cause irritation rashes, your artist will shave the area for you.
If you wish to use any numbing agents for your Tattoo, please inform your Tattoo artist before hand. We can advise of the best way to apply it to the area. We do not sell, supply or apply numbing agents and our preference is for you not to use it as it can cause the skin to toughen up or become rubbery which can make it harder to Tattoo, making it more likely that you'll need a touch up or possibly can make it harder to heal.
If you have sunburn or any skin irritation to the area, we will not Tattoo you and you will lose your booking fee.
We have limited space in the Tattoo area so please only bring one person with you to your appointment. Do not bring children or minors (under 18) to your appointment.
Please arrive to your appointment on time, your artist has allocated a specific amount of time for you, should you arrive late, your artist may not have time to finish your Tattoo which will result in you having to make another appointment to finish your Tattoo and therefore you will be charged a minimum of a £30 set up fee plus £30 for every additional half hour.
You must bring valid photo ID to your appointment, this could be a passport, drivers/provisional licence, UK validate card or citizen card, we cannot except birth certificates.
We will not Tattoo any person under the age of 18 years, as its against the law to do so.
Your booking fee secures your appointment date and time taken to draw and prepare your design, this needs to be paid within 24 hours of making your booking. Your appointment will not be confirmed until this has been paid.
If you have a gift voucher, you can use this as your booking fee to secure your appointment, you will need to provide your voucher number to do so. You may lose all or part of your voucher if you cancel without giving the required 72 hours notice.
Our artists will not copy someone Elses design, they will however design their own version or make adjustments to the original.
It is important that you are clear on the design you want before you book an appointment, please provide picture references, size, placements and include as much detail as possible to your artist.
It can take our artists hours to put together your design, making major alterations within days or on the day of your tattoo will not give them enough time to re-design your Tattoo. This may result in your artist needing to re-book you and you may lose your booking fee.
Your artist has allocated a specific amount of time for your appointment based on your chosen design and size and this reflects in the price given. So you wish to adjust or change the design So it's bigger or different the price will be adjusted and may cost more than originally quoted. If you wish to go smaller or remove parts of the design, then the price will remain the same as originally quoted.
Your Tattoo artist will discuss aftercare with you, we reccommend that you follow their advise to get the best healing results. We sell a selection of aftercare products which we have carefully selected and we recommend.
If you are going on holiday, swimming or using a hot tub within 3 weeks of getting your Tattoo, we advise that you re-book your appointment. Sun, sea, hot tubs and pools are very damaging to your Tattoo and can greatly increase the risk of infection.
If at any point you are concerned about your Tattoo, the please reach out to your artist immediately.
Here is your Tattoo aftercare advice for cling film healing but if you should have any questions please do not hesitate to contact your artist. Your artist will give you the appropriate advice for second skin healing. Please leave your covering on until you get home, remove the covering and wash with warm water and soap using just your hand and allow to air dry for at least 30 minutes. Do not submerge your tattoo for at least the first 2 weeks. It is extremely vital that you keep your Tattoo clean during the healing process. In the first few days you may experience some swelling, redness and weeping, particularly below the knee, this is normal and nothing to worry about but should you need piece of mind please contact your artist. You may also experience some scabbing but do not pick at them as this will damage your tattoo and leave scarring resulting in your tattoo needing a touch up which you will incur a fee for. Maintain usage of your aftercare balm/soap for a minimum of 2-3 times a day for 2 weeks. If you are using second skin as part of your aftercare routine then you'll need to keep the first piece on for 24 hours, soak off and clean the tattoo as instructed above. Once clean and throughly dry, at least 30 minutes, reapply your second piece of second skin covering and leave on for 4-5 days. Continue to keep clean and continue use of aftercare cream.
If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
