After that, click the 'I have completed the from' button at the bottom of the screen.

You will then receive an email with a copy of the form attached. Forward this to the minor of 16 or 17 years of age, along with a copy of your ID. The minor must physically present a valid passport, driver’s licence or government issued photo ID.
They must also present a copy of this completed digital form along with a copy of the legal guardian's ID.

This form is valid only for 16 and 17 year olds, and for the specific piercing(s) selected.

If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
