Please read and answer
I agree to pay the full amount of my deposit of anywhere from $100 to $200 dollars, depending on the artist, to Crimson Fox Tattoos via Venmo, cash, or another avenue if specified beforehand, and understand that this may be void if I chose to not show up to my appointment without prior notice.

I understand that if for whatever reason (including illness) I need to change or reschedule my appointment I will contact my tattoo artist within 48 hours of my appointment to change the date/time, I understand that if I do not do this, my deposit may be void and a new deposit is required.
I understand that before my appointment can be fully scheduled and "locked in" to the calendar, I must pay the full deposit amount and send confirmation of the deposit being sent with a screenshot, sent to crimsonfoxtattoos@gmail.com
I understand and agree to sending a confirmation e-mail of my deposit to crimsonfoxtattoos@gmail.com with the date the deposit was sent.
Venmo preferred, cash accepted as well, other payment forms accepted under special circumstances.

When sending your deposit through Venmo please put your name, the word "deposit" in the memo, and your artists name!

Please write in the box what service you'll be using to pay the deposit.
 
Please do not send your deposit through Venmo as goods or services/"turn on" for purchases. If sent through Venmo as goods or services (or turned on for purchases) you will be responsible for paying a 3% fee on top of your deposit amount.

Please specify the artist's name your deposit is going towards!
 
I understand that ALL DEPOSITS ARE NON-REFUNDABLE.
I understand if I reschedule my appointment three times I will lose my deposit or any funds put towards my tattoo on the third reschedule.

I understand if I want to reschedule after the third reschedule I will be required to pay the tattoo in full as my deposit for my next appointment.

If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
