I have confirmed a date and time with my artist for my appointment.
I understand that my deposit is non-refundable.
I understand that my deposit expires after 3 months. Holding deposits any longer is at my artist’s discretion.
I understand that my deposit is subject to forfeit if I don’t confirm my appointment by answering or returning the studio’s confirmation call/text.

I understand that not showing up for my appointment automatically forfeits my deposit and that I will be required to pay another to reschedule.

I understand that being more than 20 minutes late is counted as a no-show and will forfeit my deposit.

I understand that rescheduling or canceling my appointment without 48 hours notice automatically forfeits my deposit and that I will be required to pay another to reschedule.

I understand that rescheduling my appointment more than 2 times forfeits my deposit and that I will be required to pay another to reschedule a 3rd time.
I understand that my design is final and any changes will be made at my artist’s discretion.

I understand that the artist holds the right to reschedule me and/or forfeit my deposit if my design change is too work intensive to complete the day of my appointment.

If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
