I have had a Color Consultation PRIOR to my Color Service with the Stylist I am seeing to ensure my appointment has been booked for the proper length of time, I have been given an estimated price quote for my service and my Stylist has explained all possible outcomes for my Color.

I understand that previous treatments and chemical services can continue to have an effect on my hair. It has been recommended to me that I fully disclose my previous treatments to my stylist prior to the service. I realize that any information withheld regarding previous treatments will increase my chances of damage and the potential for unpredictable chemical reactions.

I understand that chemical processes may cause some damage to the integrity of my hair. I have been made aware of products recommended to improve the health of my hair as well as maintain the results after the service.
