Non-Refundable*
I acknowledge that my $50/hour deposit is NON-REFUNDABLE and will either be used towards my tattoo, or forfeited to the artist.
Commitment*
I acknowledge that my deposit is for the specified artist, and design. I understand that if I change my design entirely, or want to switch artists, my deposit will be forfeit to the original artist for their lost time.
Reschedule Limitations*
I understand that this deposit applies to, and allows for, only three (3) reschedules. Once an appointment has been rescheduled three (3) times the deposit is forfeit, this is to make up for the artists lost time
72-Hour Policy*
I acknowledge that I must give 72 hours notice (3 days) if I need to re-schedule my appointment. If I do not give 72 hours notice to my artist, I understand my deposit will be forfeit.
Weekends*
I acknowledge that Good Form Tattoos is closed on SUNDAY and MONDAY and I will need to either contact my artist personally on those days if I need to re-book, or add two days onto the given notice time to compensate for the weekend.
Multiple Session Tattoo*
I acknowledge that if my tattoo takes more than one session to complete, my ENTIRE deposit will remain un-used until the final session of my tattoo.
3 Month Policy*
I acknowledge that if for whatever reason I do not book my appointment at the time of leaving my deposit, I have 3 months to do so before my deposit is forfeited to the artist.
Skin Damage*
I acknowledge that if I arrive to my appointment with damaged or altered skin (THIS INCLUDES ANY AND ALL NUMBING CREAMS) to the extent that it cannot be tattooed, I understand my deposit will be forfeit to the artist for their lost time.
Right to Refuse Service*
Good Form will NOT tolerate any form of abuse, violence, or inappropriate behaviours in the studio, out outside of the studio directed at our staff or other clients. Good Form reserves the right to refuse services, and if I am refused services, my deposit will be forfeit, and I will no longer be welcome at the studio.
Good Form's goal is to provide clear expectations between the artist and client to create the best possible tattoo experience. If for any reason a clients deposit is forfeit, Good Form will make two attempts to contact the client via the e-mail and/or phone number provided below before proceeding.

If extenuating circumstances arise, the client may request special exceptions, however the approval of these exceptions must be made before the agreed upon time frame passes, and are dependent on approval from the artist and a manager at Good Form.
If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
