Agreement*
I agree that I have discussed my jewelry options with a staff member at Ahimsa Piercing Studio. I have been given the opportunity to ask questions about the piece of jewelry, and understand how the piece works. I have confirmed that the name/size/gold color and gemstones of the jewelry listed on this form are accurate. I understand that changes can not be made to my order once it has been placed.
Jewelry Acknowledgement*
I understand that gold jewelry may take 6-8 months to be completed and that I will be contacted once my jewelry has arrived to the studio. I know that gold jewelry is hand made and that when genuine stones are used slight variations may arise. I understand that BVLA has a lifetime warranty on manufacturer defaults (gem falling from setting, or the closure of the piece breaking off), but that this warranty does not cover loss of jewelry.
If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
