All tattoo appointments are subject to a $50 set up fee in addition to only the first hour or minimum rate. This includes touch-ups.

Artists Rights

The artist has the right to refuse any client and any tattoo. If the client is rude, disrespectful or abusive, the appointment will be canceled and the deposit will be VOID. If the tattoo is racist, hateful, or gang-related, the artist has the right to refuse the tattoo. If the client is rude, disrespectful, or abusive, they may be banned from receiving any services, from any artist at the studio, at any future time.

Waive*

TO WAIVE AND RELEASE to the fullest extent permitted by law each of the Artist and the Studio from all liability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors or assigns may have for personal injury or otherwise, including any direct and/or consequential damages, which result or arise from my tattoo, whether caused by the negligence or fault of either the Artist or the Tattoo Studio, or otherwise.

Bloodbourne Pathogens

I will speak to my Artist if I have any bloodborne pathogens, transmittable diseases or recent illnesses prior to the tattoo. (It' okay if you do, we just want to know for our and other's safety).

Fainting

IF I AM PRONE TO FAINTING - I will speak to my artist prior to the tattoo.

Health

I will speak to my artist if i have diabetes, epilepsy, hemophilia, a heart condition, nor do I take blood thinning medication. I do not have any other condition that may interfere with the application or healing of the tattoo. I am not the recipient of an organ or bone marrow transplant or, if I am, I have taken the preventive anti-biotics. I am not pregnant or nursing. I do not have a mental impairment that may affect my judgment in getting the tattoo.

Risks

I have been fully informed of the inherent risks, associated with getting a tattoo. I fully understand that these risks, known and unknown, can lead to injury, including but not limited to infection, scarring, difficulties in detecting melanoma and allergic reactions to tattoo pigment, latex gloves, and/or soap. Having been informed of the potential risks, I still wish to proceed with the tattoo application and I freely accept and expressly assume any and all risks.

Influence

I am not under the influence of alcohol or drugs, and I am voluntarily submitting to be tattooed by the Artist without duress or coercion.

Healing

The Artist and the Tattoo Studio have given me instructions on the care of my tattoo while it's healing, and I understand them and will follow them. I acknowledge that it is possible that the tattoo can become infected, particularly if I do not follow the instructions given to me. If any touch-up work to the tattoo is needed due to my own negligence, I agree that the work will be done at my own expense. All touch ups are subject to fee’s.

Colours and clarity

Variations in color and design may happen between the tattoo art I have selected and the actual tattoo when it is applied to my body. I also understand that over time, the colours and the clarity of my tattoo can change and fade due to various reasons. If you have any questions ask your artist prior to your tattoo.

I understand that white ink will not look white because tattoo ink is underneath the skin and the tone of my skin will affect the way white, and pale colours of ink appear.

Spelling

Neither the Artist nor the Tattoo Studio is responsible for the meaning, spelling, grammar or punctuation of the symbol or text that I have provided or had them design/draw for me.

Permanent

I understand a tattoo is a permanent change to my appearance and cannot be removed by the tattoo artist.

Photographs

I release all rights to photographs taken of me and the tattoo. I give consent to reproduction in print or electronic form.
It is okay to ask your artist to see any photos of them prior to posting during your tattoo session, If nothing is discussed prior to your tattoo you are giving permission to post or replicate any photos taken.

Sterile packages

I will witnessed all sterile packages opened and sterilization has been explained, all my questions will be answered to full satisfaction. There is a sharps container for proper disposal of my needles.

COVID SCREENING

Do you have any of the following new or worsening symptoms or signs? Symptoms should not be chronic or related to other known causes or conditions?

Difficulty breathing or shortness of breath
Cough
Sore throat, trouble swallowing
Runny nose / stuffy or nasal congestion
Decrease or loss of smell or taste
Nausea, vomiting, diarrhea, abdominal pain

Have you travelled outside of Canada in the part 14 days?
Have you had close contact with a confirmed or probable case of COVID-19?

Artists have the right to refuse any work If you have symptoms, or been overexposed to sun, are intoxicated or arrive in a state that does not allow the success of the tattoo. This will require you to rebook and forfeit your deposit.

By submitting this form you are agreeing to the terms above.

If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
